Personalised Wound Management in Elizabeth
Every participant’s wound care needs are different. Our registered nurses develop personalised wound management plans based on the type of wound, the participant’s overall health, medical history and individual circumstances. We provide nurse-led support for participants living with surgical wounds, pressure injuries, diabetic foot ulcers and other complex wounds.
Our services aim to support wound healing conditions through appropriate wound care, pressure management and monitoring, recognising that wound outcomes depend on many factors. These may include the participant’s underlying health conditions, circulation, nutrition, pressure management, continence and moisture exposure, infection risk, prescribed equipment, consistency of daily support, adherence to medical advice and ongoing clinical oversight.
By working closely with participants, families, support workers, GPs and other health professionals, we support continuity of care and help ensure wound-related changes are identified, documented and communicated promptly to facilitate appropriate clinical decision-making.
